Weekly Wellness Check

$145 for a single + Trip Fee/ $550 per month

•Vital signs check

• Nursing Assessment

•Check food & fresh water supply

o Check fridge and pantry for ample supply for the week

o Check one bathroom toilet, sink, and shower/tub for water supply.

o Check kitchen sink to see that water comes out of the spout and is not too hot to the touch

•Heat and/or cooling system check

o Check internal home temperature.

o Check that heating and cooling systems turn on.

•General safety check

o Check for major tripping hazards, obvious threats to health and safety

Medication Management

$650 for 5 visits, $1200 for 10 visits  (plus trip fee)

•Review all prescriptions for accuracy

•Pill organizer set up

•Medication Education

Daily Medication administration support (supervise medication administration)

Medication Infusion

$99/hr. 2 hr minimum.

Prorated in 15 minute increments

•Nurse administration of medication ordered by your medical provider

•Vital signs check

•IV/Midline/PICC line site assessment

 

Patient advocacy

$249 (2 hours)/$549 (5 hours)​

•2-hour assessment includes:

•review of medical history, current issues, suggested resources and solutions for care.

•5-hour package includes

•Consultation

•Coordination

•assessment

Additional hours will be billed at $250/hour for continued assistance with research, presentation of options, care coordination, and referrals for insurance advocacy, legal and financial experts.

 

Specialty Rates

Trip Fee: $1/mile (For each mile out of the boundaries of the city of San Diego)

After Hours: additional $45/hr

Holiday: additional $75/hr
